Records Request

To protect the privacy and confidentiality of client records, all requests for records from Arizona Women’s Recovery Center must be accompanied by a completed and signed Authorization for Release of Information form.

Please complete the attached Release of Information form and submit it by email to: records@azwomensrecoverycenter.org.

Requests cannot be processed until the completed authorization is received and verified. Records will be released only as authorized and in accordance with applicable federal and Arizona confidentiality requirements, including HIPAA.
